Services Coordination
What is Service Coordination?
Service Coordination is a support service that helps consumers and their families get the types of services they may need or want. A Service Coordinator works with the consumer, their family, and/or advocate to help them access social, medical, clinical, educational, vocational, residential, and other services or supports that help the consumers reach their full potential. The Service Coordinator helps the consumer make and meet goals that are important to the consumer.
What is a Consumer Choice of Service Coordinator?
The Service coordinator is chosen by the consumer to work for the consumer. One of the key components of Service Coordination is to work as a partner with the consumer to live and take part in in their community in a way that matches the consumer's needs, personal goals, and preferences

OTHER POSSIBLE SERVICES
RESIDENTIAL HABILITATION
Building skills needed for activities that take place in the consumer's home such as personal grooming and cleanliness, bed making, and household chores.
DAY HABILITATION
Day program(s) to build skills needed for activities that take place outside the consumer's home.
DAY HABILITATION WITHOUT WALLS
Building skills needed for activities that take place outside the home such as assistance with shopping community safety skills, recreational, social, and community oriented independent living skills.
ADAPTIVE DEVICES
Devices, aides, controls, supplies, or appliances, (either adaptive or for communication) that are needed to assist consumer increase or maintain their ability to live at home.
ENVIRONMENTAL MODIFICATIONS
Changes to the home area to help the consumer maintain independence at home.
RESPITE
Relief or substitute services to give a break to the primary caregiver and support the consumer.
OTHER
Other services such as transportation, medical, recreation, advocacy, educational support, housing, legal, application assistance, and counseling may be obtained for the consumer., and counseling may be obtained for the consumer.

- INTEGRATION-Belonging to a neighborhood and community, being involved with relatives and friends.
- PRODUCTIVITY-Gaining abilities to do things that add to the consumers own life and the lives of the people around them.
